We are looking for a Licensed Social Worker to join our team at NWH in our Psychiatric Unit. The Clinical Social Worker (CSW) on the psychiatric inpatient units performs psychosocial evaluations which are incorporated into a multi-interdisciplinary treatment planning process. The patient care is coordinated, and the clinical social worker is responsible to communicate and engage the patient/family and outside team in treatment/discharge planning conferences. Your primary focus will be on assessment, discharge planning crisis intervention, advocacy, information/referral, and outpatient follow up are the base of the clinical social work intervention.
The main focus of this role is in the following key areas:
* Obtains psychosocial data from patient, families and treatment providers in order to contribute to multi-interdisciplinary treatment planning.
* Formulates psychosocial assessments and develops social work treatment goals and plans.
* Identifies family issues and/or barriers to discharge; coordinates family meetings and/or treatment planning conferences with patient/family and treatment team to work on identified issues.
* Collaborates with other team members on family meetings and discharge planning.
* Provides education regarding patients illness and coordinates family/treater meetings with attending physician to discuss patients diagnosis, prognosis, and discharge planning needs.
* Develops and implements discharge planning with outside agencies.
* Maintains patient records and other documentation in a manner consistent with standards of clinical care, hospital, department, third party and regulating agencies policies and procedures, and standards of risk management.
* Attends interdisciplinary rounds to obtain input and report casework activities to other team members.
* Participates in Quality Improvement process, contributes to Departments mission by serving on task forces and committees and participating in pertinent activities.
* Works collaboratively with the UR manager in managed care reviews.
* Participates in continuing education opportunities to enhance and broaden skills relevant to job performance; maintains documentation of continuing education and in-service activities provided.
The ideal candidate will have the following qualifications and expertise:
* Master%27s degree from a graduate program accredited by the Council on Social Work Education.
* Previous experience includes work with adults and families with major mental illness, chemical dependency, and other clinical disorders identified in the DSM
* Licensed in Massachusetts at the licensed independent clinical social work level.
* Post master%27s experience (2-3 years) preferred.
